All cancellation requests must be made in writing or via this form addressed to The Gym LA, Attn: General Manager at 11567 Santa Monica Boulevard, Los Angeles, California 90025, not less than 30 days prior to date said cancellation will be effective.
Dues for the last 30 days must be paid when cancellation is submitted. Cancellation shall be deemed submitted on the date it is received by The Gym LA. Verbal cancellations will not be accepted. For your convenience you may email your request to our billing department at billing@thegym.la

CANCELLATION ELIGIBILITY

You (or Your legally appointed estate, conservator or guardian) may cancel this Agreement in the event of Your death or Your permanent medical disability upon the presentation of evidence reasonably satisfactory to The Gym LA. In the event You move further than 25 miles from the Facilities, and from all other locations of The Gym LA, You may cancel this Agreement upon written notice to The Gym LA. The Gym LA may require you to submit evidence of your relocation reasonably satisfactory to The Gym LA (e.g. a certified copy of a fully-executed lease agreement). The Gym LA reserves the right to assess a $99.00 fee (or $50.00 if more than half of the Term of this Agreement has expired) upon Our acceptance of your cancellation under this paragraph. Unless otherwise permitted herein, you may not cancel or terminate this Agreement.
